New 90-69 Guidance

New Procedure Announcement



FEMA has authorized a change in how 90-69 forms may be used.

Currently, the 90-69 document is a 4-part form. FEMA will eventually only produce a 2-part form.

Effective immediately:

  • Independent Contractors may now take the 4-part form, divide it in two, and make two 90-69 forms out of it.  
  • SEPARATE THE FORMS BEFORE ANY APPLICANT INFORMATION IS ENTERED to avoid applicant data transferring onto the copies.
This change will increase your inventory of forms and decrease your shipping costs
