New Fingerprinting Form

Many of you may have fingerprint packets in your possession that you have not completed and returned yet. Unfortunately, a form in this packet, the 12-80, has been replaced with a newer form, the 121-2-1. The new form must now be completed and accompany your returned packet. To receive the new form, please contact the Call Center at 800-411-1177.
